You could then email the document to clientcare@myhealingdynamics.com, or print it out to bring it in or if you prefer, simply print out the blank forms to handwrite in all your information. To help expedite your first visit, please bring with you, your script from your referring physician, your insurance card and a valid ID. We must also receive your completed registration forms.
